CTH - Stat dec

A simple statutory declaration (any State) required to declare that a proper process of delivery of termination of (residential) tenancy agreement has been conducted, due to non-payment. It uses a Commonwealth stat dec format which could usually be used in any State. Some ideas here may be useful if you are thinking to re-write in accordance with State/Territory stat dec requirements. (L-13031)

A 'stat dec' form for declaration in the Commonwealth of Australia for a worker who is a carer who needs to provide evidence for leave taken for medical reason(s) and who can't (reasonably) go to doctor for medical certificate but is still entitled to payment for leave under https://www.fairwork.gov.au/leave/sick-and-carers-leave/paid-sick-and-ca.... An interesting Stat Dec. (Commonwealth). The beneficiary (one of two) mentioned in the Will agrees with the Executor that to include a third beneficiary would be useful as it would mean no legal arguments further down the track. Stat dec. Any state. Certificate of experience provided by employer upon request of employee. Similar to a formal reference but this Experience Certificate is provided under a Cth statutory declaration to back up a tender or corporate requirement or for stamping by Dept of Foreign Affairs as required by UN or foreign government. A statutory declaration (Cth) for inclusion in the RFT to be filled in by a tenderer who is a single corporation and signed by an officer authorised by the Board under an Instrument of Delegation. It is for use in the special case where two tenderers have openly colluded to independently bid for and supply separate parts of the tender, the sum of which meets or exceeds the requirements, and who wish to be considered concurrently for the tender.

Another way of achieving the Personal Services Contract. A statutory declaration (any State) to accompany any contract between an Issuer and a Tenderer where the Tenderer declares that he/she qualifies as a personal services business under one or more of the four qualifications laid down by the ATO under the Alienation of Personal Services Income Regulations NAT 8028 and includes the definition of qualifications laid down by NAT 4788, thereby giving the Issuer the confidence to go ahead with signing the contract.
